Please print both pages, read page 2, complete and sign page 1 in ink, then mail both pages to the address on the form together with a cheque or bank draft.

Alternatively, if paying by bank transfer, you may prefer to send a scanned copy of the completed form to us as an email attachment.

You are advised to keep a copy of your completed form + Booking Terms & Conditions for future reference (or we can send a copy to you on request).
